Job summary

Reema Health, Inc. is seeking a Community Health Navigator (Community Guide) to build relationships with members and guide them toward greater engagement in their own healthcare.

You will work daily on Reema's platform to reach members, schedule visits, and document progress while addressing social barriers in a growing startup. As a Community Guide, you will travel 25–50% within a 30-mile community service area, maintain HIPAA confidentiality, and collaborate with a multidisciplinary team.

Qualifications

• Comfort with learning new tools on mobile and desktop.
• Self-motivated to work on a remote caseload with limited oversight.
• Collaborative mindset for remote team work and occasional in-person meetings.

Responsibilities

• Conduct outreach and enroll members in Reema's care model.
• Build and manage a caseload (80-120 members).
• Assess member needs and develop member-centered care plans.
• Coordinate referrals and schedule appointments.
• Document work in the Reema App and maintain privacy standards.
