Job summary

Reema Health in Cleveland, OH seeks a Community Health Navigator (Community Guide) to build trusted relationships with members and guide them toward engaged, healthier lives. You will work with a multidisciplinary team to address social barriers and document progress using Reema's platform.

The role requires 80–120 member caseload management, daily outreach, and strong communication skills. Travel 25–50% in a 30-mile area is expected, with a Monday–Friday schedule and competitive compensation.

Qualifications

• 18 years of age or older.
• Available to work Monday–Friday during core business hours.
• If field-based: Valid driver's license, insurance, and reliable transportation
• Offers contingent upon passing a pre-employment drug screening and background check
• Comply with federal, state, and local regulations regarding patient confidentiality and HIPAA
• Use technology-based tools and systems daily
• Lift and carry up to 25 lbs occasionally

Responsibilities

• Conduct phone and in-person outreach; enroll members in Reema's care model.
• Build and manage a caseload (80-120 members).
• Assess member needs to build a member-centered care plan.
• Coordinate referrals and scheduling; attend to follow-ups.
• Document all member work in the Reema App and keep care plans current.
• Maintain HIPAA and confidentiality standards.
