Job summary

Molina Healthcare is seeking a Community Health Worker to provide support for community-based member advocacy. You will engage as a local resource to help vulnerable members manage health care needs and navigate plan benefits.

The role involves meeting members in homes, shelters, and provider offices, assisting with appointments, transportation, prescriptions, and access to social services. Travel up to 80% may be required.

Qualifications

• 1 year health care experience with underserved populations.
• CHW certification may be required in certain states.
• Valid driver's license and reliable transportation.
• Ability to multi-task while speaking with members.
• Excellent verbal and written communication.
• Proficiency in Microsoft Office applications.

Responsibilities

• Advocate for members and help manage health care needs.
• Meet with members in homes, shelters, and provider offices for non-clinical duties.
• Assist members in navigating plan benefits, scheduling appointments, transportation, and prescriptions.
• Help members access social services such as housing, food, and employment.
• Engage in outreach to locate and assist disconnected members.
• Conduct research to locate hard-to-reach members using available data.
• Participate in ongoing or project-based member outreach efforts (phone or in-person).
• Guide members to maintain Medicaid eligibility and access financial resources.
• Travel 50-80% locally as required by state/contractual requirements.
