Job summary

Travel will be 50-80% locally to meet members, providers, and community resources, contributing to strategy for quality, cost-effective care. Strong communication and MS Office proficiency are required.

Qualifications

• Minimum 1 year of health care experience with underserved populations.
• CHW certification may be required for certain states.
• Valid driver's license and reliable transportation for job travel.
• Ability to multitask applications while interacting with members.
• Excellent customer service and communication skills; proficiency with basic software systems.

Responsibilities

• Advocate for members and assist with managing health care needs.

• Help navigate plan benefits, schedule appointments, and arrange transportation.
• Assist members in accessing social services and community resources.
