Job summary

Molina Healthcare is seeking a Community Health Worker to support community-based member advocacy activities in Ohio. This role involves engaging with members in their communities, coordinating with the health care services team, and helping navigate Medicaid and health plan benefits.

Extensive local travel (50-80%) may be required to meet members in homes, shelters, and provider offices, with a focus on underserved populations and facilitating access to social services and resources.

Qualifications

• Minimum 1 year of health care experience with underserved populations.
• CHW certification may be required for some states.
• Valid driver's license and reliable transportation for local travel.
• Ability to multi-task applications while speaking with members.

Responsibilities

• Engages with members as an advocate and resource to support management of health care needs.
• Collaborates with and supports the health care services team by providing non-clinical paraprofessional duties in the field.
• Empowers members by helping them navigate and maximize their health plan benefits.
• Assists members in accessing social services such as housing, food, employment, etc.
• Provides outreach to locate and support disconnected members with special needs.
• Conducts research to locate members Molina has been unable to contact.
