Molina Healthcare in Ohio is seeking a Community Health Worker to support local member advocacy activities. You will serve as a local member advocate and resource, using knowledge of the community to engage and assist vulnerable members in managing health care needs.

The role involves home visits, collaboration with the health care services team on non-clinical duties, helping members navigate benefits, arrange transportation, access social services, and maintain Medicaid eligibility.

Qualifications

• Healthcare experience in underserved populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job travel.
• Ability to multi-task applications while speaking with members.
• Excellent customer service skills.
• Organizational and time-management skills.
• Ability and willingness to learn other lines of business, programs and relevant software systems/applications.
• Excellent verbal and written communication skills.
• Microsoft Office suite/applicable software proficiency.

Responsabilités

• Engage with members as an advocate and resource to support health care needs.
• Collaborate with the health care services team with non-clinical paraprofessional duties in the field.
• Help members navigate and maximize plan benefits; assist with appointments and transportation.
• Assist members in accessing social services such as housing, food, employment.
• Provide outreach to locate and assist disconnected members with special needs.
• Conduct outreach and research to locate hard-to-reach members.
• Participate in projects requiring extensive member outreach.
• Guide members to maintain Medicaid eligibility and financial resources.
• Travel locally 50-80% as required.
