Formación

• At least 1 year of health care experience serving underserved populations.
• CHW certification may be required depending on state.
• Valid driver's license, reliable transportation, and adequate auto insurance.
• Excellent customer service and communication skills.
• Ability to multi-task while engaging with members.

Responsabilidades

• Engages with members as an advocate and resource to support health care needs.
• Collaborates with the health care services team by performing non-clinical paraprofessional duties in the field.
• Helps members navigate and maximize health plan benefits, including scheduling and transportation arrangements.
• Assists members in accessing social services such as housing, food, and employment resources.
• Travel up to 50-80% locally as required by state/contract.
• Conducts outreach to locate and assist disconnected members.
