Job summary

• Knowledge of community resources and social services is a plus.

Responsibilities

• Advocate for members and connect them with health plan resources.
• Meet with members in various settings (homes, shelters, clinics) as needed.
• Assist with navigating health plans, scheduling, transportation, and prescriptions.
• Help members access social services such as housing, food, and employment resources.
• Reach out to locate disconnected members and provide support for special needs.
• Conduct research to locate hard-to-reach members using available data.
• Engage in outreach activities, in-person or by phone, per project requirements.
• Support Medicaid eligibility maintenance and related financial resources.
