Job summary

Molina Healthcare is seeking a Community Health Worker to support community-based member advocacy. You will meet members in homes, shelters, and offices to help them understand and access health plan benefits, coordinate care, and navigate social services.

The role involves substantial local travel and collaboration with care teams. Required qualifications include health care exposure and a CHW certification where applicable, plus strong communication and organizational skills.

Qualifications

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required in certain states.
• Valid and unrestricted driver's license and transportation for travel.
• Ability to multi-task while speaking with members.
• Excellent customer service and communication skills.
• Proficiency with Microsoft Office and applicable software.

Responsibilities

• Engages with members as an advocate and resource to manage health care needs.
• Collaborates with the health care services team and performs paraprofessional duties in the field.
• Helps members navigate health plan benefits and arrange transportation or appointments.
• Assists members in accessing social services and community resources.
• Conducts outreach to locate and support disconnected members with special needs.
• Performs outreach and research to locate hard-to-reach members.
• Participates in outreach activities, telephonic or in person.
• Guides members to maintain Medicaid eligibility and other resources.
