Job summary

Molina Healthcare is seeking a Community Health Worker in Spokane, WA to advocate for members and connect them with health care resources. The role requires in-field outreach, home visits, and collaboration with the care team to ensure access to benefits and services.

You will help members navigate Medicaid eligibility, arrange transportation, and coordinate with providers. A CHW certification may be needed for some contracts, and substantial local travel is expected.

Qualifications

• At least 1 year of health care experience, preferably with underserved or special needs populations.
• CHW certification may be required for certain states.
• Valid driver’s license, reliable transportation, and adequate auto insurance for job-related travel.
• Ability to multi-task applications while speaking with members.
• Excellent customer service skills.
• Organizational and time-management skills.
• Ability and willingness to learn other lines of business, programs and relevant software systems/applications.
• Excellent verbal and written communication skills.

Responsibilities

• Engage with members as an advocate and resource to support health care management.
• Collaborate with the health care services team providing non-clinical paraprofessional duties in the field (home visits, shelters, provider offices).
• Help members navigate and maximize plan benefits (scheduling, transportation, prescriptions, follow-ups).
• Assist members in accessing social services and community resources.
• Provide outreach to locate/disconnected members with special needs.
• Conduct data-driven outreach to locate hard-to-contact members.
• Travel 50–80% locally as required by state/contractual rules.
• Support Medicaid eligibility maintenance and related financial resources.
• Participate in ongoing or project-based outreach activities (telephonic or face-to-face).
