Qualifications

• Must travel within 90 mile radius from Champaign County, IL at least 70% of the time.
• Must possess reliable transportation.
• 1-3 years of relevant experience (community outreach or home visits).
• Experience serving Medicaid populations in urban or rural environments with local resource networks.

Responsabilités

• Serve as a bridge between members and the health care system through outreach and education.
• Help members attain and maintain better health outcomes and improve relationships with providers.
• Communicate medical information to health care professionals and care coordinators by telephone.
• Work with a diverse community, using knowledge of local formal and informal resources.
• Travel to multiple office locations and member homes as required.
