Qualifications

• Reside in LA and within assigned region.
• Able to travel 60–75% of the time and have reliable transportation.
• 1–2 years of clinical support experience.
• 1 year of community outreach experience and knowledge of LA resources.
• 1 year experience with clients with clinical needs (physical/behavioral health).
• Proficient with MS Office Suite and EHR systems.

Responsabilités

• Conduct home visits with members to reinforce care plans and adapt tasks to culture and language.
• Educate providers on social determinants of health affecting care.
• Build and maintain relations with members, providers, nurses, and staff from diverse backgrounds.
• Assist with care plan implementation and health management planning.
• Identify socio-economic barriers and connect members to community resources.
• Enter and maintain member records in EHR and complete documentation.
• Travel extensively to client homes and outreach destinations.
