Job summary

CVS Health Corporation is seeking a Field-based Community Health Worker to serve Lake County, IL and surrounding counties. This full-time role requires up to 70% travel, supporting outreach, education, and care coordination with providers to improve health outcomes.

The CHW will engage with Medicaid populations in diverse environments, traveling to member homes and clinics, while maintaining strong communication and organizational skills.

Qualifications

• Must be located in Lake County, IL or surrounding area.
• Must be willing to travel within a 90 mile radius from Lake County, IL at least 70% of the time.
• Must possess reliable transportation.
• 1-3 years relevant experience in community outreach or home visits.
• Experience serving Medicaid populations in urban or rural environments.

Responsibilities

• Serve as a bridge between members and the health care system through outreach and education.
• Travel to multiple office and home locations; communicate medical information to health care professionals by telephone.
• Collaborate with care coordinators and providers to adjust interventions based on needs.
• Provide support to socially and medically complex patients in various settings.
