Job summary

CVS Health is hiring a Community Health Worker to serve Louisiana Region 1. The role conducts in-home visits, educates members on care plans, and connects them with community resources while traveling up to 70% of the time.

The CHW will build relationships with diverse communities, assist with care management, and document interactions in an electronic health record. Requires strong communication and reliability; 40-hour weeks with some evenings.

Qualifications

• Travel 70% of the time; must have reliable transportation.
• 3–5 years' relevant work experience.
• 1–2 years' experience providing clinical support services.
• 1 year community outreach experience and knowledge of LA resources.
• 1 year experience working with clients with clinical needs (physical and/or behavioral health).
• Proficiency with a personal computer, keyboard navigation, and MS Office Suite applications and EHR systems.

Responsibilities

• Conduct home visits with members to reinforce care plans and reduce hospitalizations.
• Educate healthcare professionals on social determinants of health.
• Build relationships with members, providers, and staff from diverse backgrounds.
• Work to reduce cultural and socio-economic barriers between clients and institutions.
• Assist with care plan implementation and member education during in-home visits.
