Qualifications

Responsibilities

• Educate providers on social determinants of health and care improvements.
• Build relationships with members, providers, and care teams from diverse backgrounds.
• Assist with care plan implementation and health/social management planning.
• Identify socio-economic issues affecting overall health and coordinate community services.
