Molina Healthcare is seeking a Community Health Worker in Ohio to provide in-home and field-based member advocacy, helping vulnerable individuals access healthcare and social services. You will collaborate with care teams, schedule appointments, arrange transportation, and assist with Medicaid eligibility navigation.

The role involves 50-80% local travel and requires strong communication and customer service skills. Certification as CHW or CMA is preferred in some states.

Formación

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver's license and reliable transportation.
• Ability to multitask while communicating with members.
• Excellent customer service, organizational and time-management skills.
• Willingness to learn other programs and software systems.
• Strong verbal and written communication skills.
• Proficiency in Microsoft Office applications.

Responsabilidades

• Advocate for members and serve as a local resource for healthcare needs.
• Provide non-clinical paraprofessional duties in homes, nursing facilities, shelters, etc.
• Help members navigate health plan benefits and arrange appointments/transportation.
• Assist in accessing social services and community resources.
• Reach out to locate disconnected members with special needs.
• Conduct outreach and member follow-up, including telephonic or in-person contact.
• Support Medicaid eligibility maintenance and financial resources guidance.
• Travel 50-80% locally as required.
