Oak Street Health is hiring a Community Health Worker in Glendale, AZ. The CHW will serve as a patient advocate and liaison, expanding access to health services and community resources.

You will work outside the center at various community sites, assisting patients with applications for benefits and coordinating care with the medical team. The role requires at least 1 year in healthcare or social services, strong communication, and reliable transportation.

Formación

• Minimum of 1 year of healthcare, community-based, case management, or social service experience.
• Strong oral and written communication skills.
• Ability to manage multiple priorities while maintaining a positive attitude.

Responsabilidades

• Establish and maintain relationships with patients, community organizations, and care team.
• Coordinate patient needs and referrals as defined by the care team.
• Facilitate communication among patients, families, providers, and community partners.
• Assist patients with accessing benefits and local resources.
• Document interactions in the electronic medical record with HIPAA compliance.
