Oak Street Health seeks a Community Health Worker to bridge patients with community resources and medical teams, promoting health literacy and access to services. You will perform patient-centered outreach and referrals, working closely with Medical Social Workers to coordinate care plans.

Strong communication, problem-solving, and community knowledge are essential. A valid US work authorization and reliable transportation are required.

Qualifikationen

• Minimum of 1 year in healthcare, community-based, case management, or social services.
• Strong oral and written communication skills.
• Ability to manage multiple priorities while maintaining a positive attitude.
• Dedication to serving the community and building relationships.
• Proficient computer skills (Windows, GSuite, Microsoft, etc.).
• Access to reliable transportation and ability to travel within the community.
• US work authorization is required.

Aufgaben

• Establish and maintain relationships with patients, community orgs, and team members to coordinate needs.
• Manage patient referrals defined by the care team and collaborate with the Medical Social Worker.
• Facilitate communication among family, caregivers, medical providers, and community orgs.
• Build inventory of local community organizations benefiting patients.
• Connect patients to housing, transportation, food, and daily living resources.
• Assist with applications for eligible benefits and resources.
• Promote goal setting and self-efficacy with patients as agreed by the care team.
• Meet patients at Oak Street Health centers, homes, or other locations as needed.
• Spend ~half time outside centers in community locations; reliable transportation required.
• Support engagement with high-risk individuals and accompany to appointments as needed.
• Complete referrals to organizations and agencies; document interactions in EMR.
• Ensure HIPAA compliance and confidentiality in all records.
