Oak Street Health is hiring a Community Health Worker in Chicago to bridge patients with medical systems and community resources. You will support health literacy, coordinate services, and advocate for patients in person and by phone.

The role requires strong communication, cultural competency, and ability to travel around the community. A CHW certification or related degree is preferred, with full-time hours and a comprehensive benefits package.

Formación

• Minimum of 1 year of experience in healthcare, community-based, case management, or social service environment.
• Strong oral and written communication skills.
• Ability to manage multiple priorities while maintaining a positive attitude.
• Dedication to serving the community and building meaningful relationships.

Responsabilidades

• Establish and maintain strong relationships with patients, community organizations, team members, and partners to coordinate patient needs.
• Manage patient referrals defined by the care team and collaborate with the Medical Social Worker on action plan.
• Facilitate communication between all identified parties involved in patients’ care as needed.
• Build an inventory of local community organizations to benefit patients.
• Connect patients to state and local resources related to housing, transportation, food, and daily living barriers.
• Assist patients with completing applications for accessing eligible benefits and resources.
• Promote goal setting and achievement to improve patients’ quality of life.
• Meet with patients in patient-centered locations such as Oak Street Health center or home.
• Spend about half of time outside the center in community locations; reliable transportation is required.
