Job summary

Qualifications

• At least 1 year of healthcare/community-based experience.
• Excellent oral and written communication skills.
• Ability to manage multiple priorities with a positive attitude.
• Proficient computer skills (Windows, GSuite, Microsoft).
• Access to reliable transportation and ability to travel within the community.
• US work authorization.

Responsibilities

• Establish and maintain relationships with patients, community orgs, and team members to coordinate needs.
• Manage patient referrals with the care team and action plans.
• Facilitate communication among patients, families, providers, and community groups.
• Build relationships with local community organizations for patient benefits.
• Connect patients to housing, transportation, food, and daily living resources.
• Assist patients with applications for eligible benefits and resources.
• Promote goal setting and health outcomes with the care team.
• Meet patients at Oak Street Health centers, homes, or community locations.
• Expect to spend roughly half the time outside the center in community settings.
• Drive engagement with high-risk individuals and accompany to appointments as needed.
• Complete referrals to organizations and agencies.
• Deliver culturally appropriate health education.
• Support care team decision making in interdisciplinary meetings.
• Document patient interactions in EMR per HIPAA standards.
• Manage time, priorities, and work independently.
• Other duties as assigned.
