Job summary

Oak Street Health is seeking a Community Health Worker in the Denver area to serve as a patient advocate and liaison, helping to remove barriers and improve access to resources. You will conduct proactive outreach, support care plans, and coordinate with the care team to promote health literacy and equitable care.

The role emphasizes in-person and outreach work, strong communication, and collaboration with medical social workers to enhance patient outcomes while adhering to HIPAA standards.

Qualifications

• Minimum 1 year in healthcare or related community services.
• Strong oral and written communication skills.
• Access to reliable transportation to travel in the community.
• US work authorization.
• Bilingual English/Spanish is a strong plus.

Responsibilities

• Establish and maintain relationships with patients and community partners.
• Coordinate patient referrals with the care team.
• Facilitate communication among patients, families, providers and community organizations.
• Connect patients to housing, transportation, food and resources.
• Assist patients with applications for accessing eligible benefits.
• Document interactions in the electronic medical record with HIPAA compliance.
• Spend roughly half of time outside the center in patient-centered locations.
