Job summary

AbsoluteCare Medical Center & Pharmacy is seeking a Community Health Worker to connect members with resources and guide enrollment in services. The role involves non-clinical assessments, building trust, and coordinating with care teams to meet engagement KPIs.

You will travel locally to members’ homes and community sites, using CRM to document outreach and support. A valid driver’s license and 2+ years in community health are required.

Qualifications

• High school diploma or equivalent required.
• Must have a valid driver’s license, proof of insurance, and reliable personal vehicle.
• 2+ years of experience with community agency providing services encompassing basic competency areas of health care.
• Certified as a CHW preferred.
• Ability to travel locally at minimum 80% of the time.
• Proficient in PC skills, such as Microsoft Office.
• Knowledge of local community resources and navigating community resource.

Responsibilities

• Represent AbsoluteCare and communicate services, connections and resources to members.
• Conduct in-person non-clinical assessments to understand member needs and barriers to care.
• Connect members with AbsoluteCare and community resources like mental health, housing and substance abuse support.
• Build trust and rapport with members and their support systems to drive engagement.
• Partner with care team and attend daily huddles to prioritize members and caseloads.
• Document daily outreach activities into CRM and medical records; meet KPIs.
