Job summary

AbsoluteCare Medical Center & Pharmacy is seeking a Community Health Worker to connect members with resources addressing social needs and barriers to care. You will meet members wherever they are—at home, in the community, or at our center—and guide enrollment in internal and external services.

The role operates in a matrixed environment with dual reporting to the Community Team Care Manager and the Engagement Director, including travel across Northeast Ohio for outreach and programs oversight.

Qualifications

• High school diploma or equivalent required.
• Must have a valid driver’s license, proof of insurance, and reliable personal vehicle.
• 2+ years of experience with community agencies providing health care-related services.
• CPR/BLS certification required.
• Certified as a Community Health Worker prior to employment.
• Willingness to travel locally at least 80% of the time.

Responsibilities

• Represent AbsoluteCare and communicate services and resources to members.
• Conduct in-person non-clinical assessments addressing social determinants of health (SDOH).
• Create and administer individualized care plans with clinical input as needed.
• Connect members with AbsoluteCare and community resources (mental health, housing, transportation, etc.).
• Build trust and rapport with members and support networks.
• Manage caseloads, enroll members in services, and ensure follow-through on plans.
• Partner with care team, attend daily huddles and team meetings.
• Document outreach activities in Jiva, eCW, and CRM systems.
• Meet KPIs for member engagement and enrollment.
• Support members in the Member Lounge with events.
