Job summary

The Community Health Worker at AbsoluteCare Medical Center & Pharmacy connects members with community resources, supports enrollment, and builds trust in diverse settings—from homes to clinics. The role focuses on addressing social barriers to care while guiding members through AbsoluteCare services.

Candidates should have a high school diploma, a valid driver’s license with reliable vehicle, and at least two years of community-based experience; CHW certification is preferred.

Qualifications

• High school diploma or equivalent required.
• Must have a valid driver’s license, proof of insurance, and reliable personal vehicle.
• 2+ years of experience working with a community agency providing services encompassing health care basics.
• Certified as a CHW preferred.
• Experience with vulnerable, hard-to-engage populations.
• Independent and persistent self-starter.
• Proficient in PC skills, such as Microsoft Office.
• Ability to travel locally at minimum 80% of the time.
• Knowledge and trusting relationships within local community.

Responsibilities

• Represent AbsoluteCare and communicate services, resources and benefits to members.
• Conduct in-person non-clinical assessments to understand needs and barriers to care.
• Connect members with appropriate AbsoluteCare and community resources.
• Build trust and rapport with members and their support networks.
• Collaborate with care team and attend daily huddles to prioritize caseloads.
• Pursue innovative ways to engage members and prevent loss to follow-up.
• Document outreach activities in CRM and health records.
• Meet predefined engagement KPIs for the role.
• Accompany members to primary care visits to improve outcomes and reduce hospital stays.
• Assist in events in the Member Lounge.
