Qualifications

• Travel up to 70% of time and access to reliable transportation.
• 3–5 years of relevant work experience and 1–2 years of clinical support.
• 1 year of community outreach experience and knowledge of LA resources.
• Proficiency with MS Office and EHR systems.

Responsibilities

• Conduct home visits and coordinate care plans across diverse communities.
• Educate providers and members on social determinants of health.
• Build relationships with members, providers, and staff from varied backgrounds.
• Assist with care plan implementation and health management strategies.
• Identify social and economic barriers to improve health outcomes.
• Enter and maintain records in electronic health systems; travel as required.
