Job summary

AbsoluteCare is seeking a Community Health Worker who will connect members with resources to address social needs and barriers to care. You will educate new members about AbsoluteCare and guide enrollment in services, working across the care management and engagement teams in a matrixed environment.

The role requires CHW certification, CPR/BLS, and a valid driver’s license. Experience with vulnerable populations and strong Microsoft Office skills are a plus. Travel locally is expected.

Qualifications

• High school diploma or equivalent is required.
• CPR/BLS certification is required.
• Certified Community Health Worker certification is required prior to employment.
• Experience working with vulnerable, hard-to-engage populations is valued.

Responsibilities

• Represent AbsoluteCare and communicate services and resources to members.
• Conduct in-person assessments of member needs and barriers to care.
• Create and administer individualized care plans for members.
• Connect members with community resources for housing, transportation, food, and more.
• Build trust and rapport to drive engagement.
• Manage a caseload and ensure follow-through on care plans.
• Document outreach activities in care management platforms and EHRs.
