AbsoluteCARE Medical Center & Pharmacy is seeking a Community Health Worker to engage with members across Cleveland communities. You will conduct in-person assessments, develop care plans, and connect members to essential resources to address social needs and barriers to care.

The role requires a valid Ohio CHW certification, CPR/BLS, and a willingness to travel locally at least 80%. This position reports to the Community Team Care Manager and the Engagement Director.

Qualifikationen

• High school diploma or equivalent.
• 2+ years of experience with community agencies providing health care related services.
• CPR/BLS certification required.
• Certified as a Community Health Worker prior to employment.

Aufgaben

• Represent AbsoluteCare and explain services and resources to members.
• Conduct in-person assessments to identify needs, barriers, and social determinants of health.
• Create individualized care plans with input from the clinical team.
• Connect members to community resources such as housing, transportation, food, and mental health services.
• Build trust and rapport with members and their support networks to drive engagement.
• Manage a caseload, enroll members in services, and ensure follow-through on plans.
• Collaborate with care team through daily huddles and meetings.
• Document outreach activities in care management platforms and EHRs.
• Meet predefined KPIs for member engagement and enrollment in programs.
• Support members in person at the Member Lounge with events.
