Job summary

The Community Health Worker at AbsoluteCARE Medical Center & Pharmacy will connect members with resources, address social needs, and build trust in home, community, hospital, or clinic settings. You will enroll new members and guide them through services, reporting to the Community Team Care Manager and Engagement Director.

This role involves in-person assessments, care plan development, coordinating with care teams, documenting outreach in Jiva and eCW, and meeting KPIs for member engagement

Qualifications

• High school diploma or equivalent is required.
• Must have a valid driver’s license, proof of insurance, and a reliable personal vehicle.
• 2+ years of community-based healthcare experience or equivalent.
• CPR/BLS certification required.
• Certified as a Community Health Worker prior to employment.

Responsibilities

• Represent AbsoluteCare and explain services and resources to members.
• Conduct in-person non-clinical assessments of member needs and barriers to care.
• Create and administer individualized care plans with clinical input.
• Connect members with community resources such as housing, transportation, and basic needs.
• Build trust and rapport with members and their support networks.
• Manage a caseload, enroll members in services, and ensure follow-through.
• Partner with care team, attending daily huddles and meetings.
• Document outreach in care management platforms and EHR/CRM systems.
• Meet KPIs for member engagement and enrollment into programs.
• Support members in the Member Lounge with events.
