Job summary

MC Community Services is seeking an experienced case manager to join our Section 17 adult services team. This role involves working in the community with adults living with serious mental illness, performing assessments, coordinating services, and ensuring supports are effective and in place.

You will conduct biopsychosocial assessments, connect clients to community resources, build trusting relationships, coordinate care across providers, monitor outcomes, and maintain thorough documentation.

Qualifications

• MHRT/C certification is required.
• Experience in case management, community integration, or similar behavioral health work.
• Comfort working independently in the field with homes and community settings.
• Strong organizational skills and thorough documentation.

Responsibilities

• Conduct biopsychosocial assessments and evaluate each person’s needs.
• Identify and connect clients to community services, natural supports, and resources.
• Build strong, consistent relationships through assertive engagement.
• Coordinate care across providers, housing, and community programs.
• Monitor whether services are working and adjust when they’re not.
• Maintain thorough documentation that supports continued care.
