True Mental Health Services is seeking a Community Case Manager to provide thorough case management in home and community settings. You will assess needs, develop service plans, connect clients to resources, and coordinate care with families, schools, and medical providers in a mission-driven, field-based role.

Qualifications

• Bachelor’s Degree in a behavioral health field.
• Valid Texas driver’s license and reliable transportation may be required.

Responsibilities

• Conduct service needs assessments and ongoing client evaluations.
• Develop, implement, and review individualized service plans.
• Connect clients with community resources and support services.
• Coordinate care with providers, families, schools, medical professionals, and community agencies.
• Advocate for clients to obtain needed services and benefits.
• Monitor client progress and adjust services as needed.
• Assist with discharge planning and aftercare coordination.
• Maintain accurate clinical documentation and case notes within required timeframes.
• Participate in daily team meetings and case reviews.
• Complete chart audits and ensure Medicaid compliance standards are met.
• Serve as a liaison between clients, families, referral sources, and providers.
• Support clients experiencing psychiatric, substance use, or crisis-related challenges.
