Job summary

Services for the UnderServed is seeking a dedicated Case Manager in Brooklyn, NY. This role focuses on providing comprehensive case management within a Certified Community Behavioral Health Clinic (CCBHC) to serve vulnerable populations with complex needs.

Field work dominates the schedule, with travel required and occasional crisis response. Responsibilities include developing service plans, linking clients to medical, behavioral health, and social services, and advocating for client needs

Qualifications

• Bachelor's Degree in Human Services or related field.
• 2 years’ experience with individuals with mental illness, substance abuse issues and/or a history of homelessness.
• Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System.

Responsibilities

• Provide case management to assigned caseload.
• Develop and monitor individual service plans in conjunction with agency consumers.
• Assist agency consumers with accessing needed community services through the development of a referral network and follow up.
• Assist agency consumers with problem solving and crisis intervention.
• Accompany agency consumers to medical, psychiatric, and public benefit appointments as needed.
• Advocate for consumers and encourage self-advocacy.
• Identify daily living skills issues and provide hands-on assistance or referrals.
• Maintain strong relationships with therapeutic support services to ensure quality care.
• Document all services in compliance with agency standards.
• Collaborate with the social service team on off-site recreation, socialization, educational and skill-building activities.
• Liaise with community-based services and organizations.
• Participate in supervision, meetings, case conferences, and trainings.
• Adhere to on-call system for crisis intervention and emergencies.
• This role comprises 75% field work; travel is required; driving not required.
• Perform other duties as assigned.
