Job summary

True Mental Health Services in Dallas is seeking a Community Case Manager to provide comprehensive case management for individuals facing mental health and behavioral health challenges in home and community settings.

You will conduct needs assessments, develop and monitor individualized service plans, connect clients with community resources, coordinate care with families, schools, and providers, advocate for services and benefits, and maintain accurate documentation.

Qualifications

• Bachelor’s degree in a behavioral health field (or related).
• Valid Texas driver’s license with clean driving history.
• Ability to pass criminal background screenings.
• Experience providing services in homes or community settings preferred.

Responsibilities

• Conduct needs assessments and develop individualized service plans.
• Connect clients with community resources and support services.
• Coordinate care with families, schools, medical professionals, and community agencies.
• Advocate for clients to obtain needed services and benefits.
• Maintain accurate clinical documentation and case notes; ensure Medicaid compliance.
