True Mental Health Services is seeking a dedicated Community Case Manager to help individuals navigate mental health and community resources in Fort Worth, TX. The role is full-time, field-based, requiring travel across local communities and homes.

You will assess needs, develop care plans, connect clients with services, and advocate for access to benefits. Qualified candidates bring compassion, organization, and the ability to work independently in diverse settings.

Qualifications

• Bachelor’s degree or higher in a behavioral health field is required.
• Valid Texas driver’s license free of major violations.
• Reliable transportation with proof of insurance.
• Ability to pass criminal background screenings.
• Comfortable working in homes and community settings.

Responsibilities

• Conduct service needs assessments and client evaluations.
• Develop and review individualized service plans.
• Connect clients with community resources and supports.
• Coordinate care with providers, families, schools, and community agencies.
• Advocate for clients to obtain needed services and benefits.
• Monitor client progress and adjust services as needed.
• Assist with discharge planning and aftercare coordination.
• Maintain accurate clinical documentation and case notes within required timeframes.
