Job summary

Humana, Inclusa team, seeks a Case Manager/Community Resource Coordinator (CRC) to serve Wisconsin Family Care members. You will partner with a Health and Wellness Coordinator to assess needs, create care plans, and support social, housing, and employment goals in the member’s setting.

Travel up to 40% in Jefferson County, WI, with mileage reimbursement. Role requires a 40-hour work week, and you may be eligible for Humana benefits and TB screening as part of our health program.

Qualifications

• Minimum Bachelor’s degree in Human Services or related field.
• If bachelor’s in other field, must have at least 3 years of FC-related experience.

Responsibilities

• Perform face-to-face social assessments for members at enrollment and every 6 months.
• Coordinate quarterly in-person visits and monthly calls.
• Develop and review member plans of care and support social, housing, employment topics.
