Job summary

Humana Inc. invites applications for a Case Manager serving the Wisconsin Family Care program in the Ashland/Superior area. You will conduct assessments, create a comprehensive care plan, coordinate with a Field Care Nurse, and arrange non-medical supports to promote member well-being.

Responsibilities include ongoing assessments, quarterly visits, and consistent communications. Remote work with travel up to 40% may be required; driving credentials and TB screening are part of the role.

Qualifications

• Bachelor's degree in human services or related field required with at least 1 year of experience serving frail elders or adults with disabilities.
• Alternative: Bachelor's in another field with at least 3 years of such experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs within MCP.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Perform quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical support services including housing, employment, and community resources.
• Maintain accurate documentation including MCP updates and service authorizations.
