Responsibilities

• Partner with Director of Care Solutions Center and Clinical team Leader assisting RNs, LPNs, Social Workers, and Admissions Assistants in hospitals, homes and alt home settings.
• Visit patients and families, represent Reserve Care, perform assessments, provide medication education and care for physical needs.
• Identify issues, prepare action plans, and contribute to quality assurance programs.
• Conduct needs assessments, performance reviews, capacity planning, and cost-benefit analyses; define user requirements and standards.
• Build relationships with hospital staff as a trusted partner.
• Assist in guiding teams to support families and patients with empathy and confidence.
• Collaborate on training programs for field staff; reinforce procedures and workflows.
• Observe and evaluate staff performance and ensure quality standards; participate in meetings and training.
