Job summary

Hospice of the Western Reserve, Inc. seeks a field-based supervisor to oversee patient-facing staff and ensure compassionate care outside the core team. You will help with admissions, visits, and education while maintaining strong relationships with families and hospitals.

You will work with clinical leaders to optimize Care Solutions Center operations, supervise staff, and drive care quality. A valid license and nursing background are essential.

Qualifications

• High School Diploma or equivalent required.
• LPN or RN required.
• Minimum 2–4 years’ experience supervising and leading teams.
• Hospital based nursing experience preferred.
• Hospice and palliative care experience preferred.
• Maintain a valid drivers' license and auto insurance for fulfilling business needs.
• Must understand critical success factors of care services.
• Excellent customer service and relationship-building skills.
• Exceptional written and verbal communications skills.
• Detail oriented with strong organizational and time-management abilities.

Responsibilities

• Partner with Director of Care Solutions Center and Clinical team Leader assisting RNs, LPNs, Social Workers and Admissions Assistants in hospitals, homes and alt settings.
• Visit patients and families, representing Reserve Care with a goal of assessments, education and training about medications and care for physical needs.
• Identify and resolve problems, prepare action plans, and manage quality assurance programs.
• Assist to carry out Care Solutions Center strategies via needs assessments, performance reviews, and capacity planning.
• Build relationships with hospital staff; act as a trusted partner.
• Train and guide teams to provide empathetic support for families and patients.
• Support and implement training programs for field staff and admissions personnel.
• Contribute to organizational strategic plans and reviews with data and analysis.
