Job summary

Reserve Care, formerly Hospice of the Western Reserve, seeks a qualified manager to oversee field staff delivering patient-centered care. The role combines supervision of nursing and support staff, care coordination across hospital and home settings, and training for teams to sustain quality standards.

You will collaborate with clinical leadership to ensure seamless admissions, assessments, and patient education, while maintaining exceptional family support and care continuity.

Qualifications

• High School Diploma or equivalent.
• LPN or RN required.
• Minimum 2–4 years’ experience supervising and leading teams.
• Hospital based nursing experience preferred.
• Hospice and palliative care experience preferred.
• Maintain a valid drivers' license and auto insurance.
• Must understand critical success factors of the Care Solutions Center.
• Excellent customer service skills and relationship building with referral sources and families.
• Exceptional written and verbal communications skills.
• Ability to relate to others and exhibit compassion to all parties involved.
• Demonstrated active listening skills.
• Detail oriented with strong organizational, time management, and prioritization skills.
• Ability to learn systems, processes, and procedures and adapt to change.

Responsibilities

• Visit patients and their families, performing assessments and providing education on medications.
• Oversee field staff who see patients outside the primary care team.
• Support Care Solutions Center with operational strategies, needs assessments, and capacity planning.
• Build trusted relationships with hospital staff and referral sources.
• Guide teams in providing compassionate care to families and patients.
• Collaborate with trainers to implement comprehensive staff training.
• Identify efficiency and customer service improvements and monitor service quality.
• Ensure adherence to procedures and maintain high standards of care and documentation.
