Hospice of the Western Reserve (Reserve Care) is seeking a Registered Nurse (RN) Clinical Team Manager to lead a transdisciplinary hospice team at West Campus, Fairview Park, OH. The role involves clinical leadership, program oversight, and collaboration with hospitals and the medical community.

The position requires an RN license, a BSN preferred, hospice or palliative care experience, supervisory background, and strong analytical skills.

Qualifications

• RN license required with clinical nursing background.
• BSN degree preferred; relevant leadership experience valued.
• Hospice/palliative care experience preferred.
• Supervisory and team leadership experience required.
• Independent and analytical thinker with good communication.
• Proficiency in Microsoft Office applications.

Responsibilities

• Provide general management and leadership of a transdisciplinary hospice team.
• Coordinate and supervise clinical staff, schedule, and assignments.
• Direct patient and family care as appropriate and may include home visits.
• Lead care conferences and resolve patient/family care issues.
• Ensure regulatory compliance and QA processes.
• Collaborate with hospitals and external providers; represent the program.
• Oversee budget and resources; promote high-quality service.
