Job summary

LifeKey Health LLC seeks a seasoned Client Relations Manager to grow our private-pay, long-term care and Medicaid waiver home care client base. This boots-on-the-ground role builds referral networks with hospitals, facilities, case managers, and physicians to deliver quality services.

You will conduct regular visits, present services, manage a robust pipeline, and close 3–5+ new client conversions monthly with a base hourly pay of $20 plus uncapped commission, benefits, and mileage reimbursement.

Qualifications

• 3–5 years in home care, home health, or healthcare sales.
• Established professional relationships and networks for referrals.
• Proven ability to generate and close new client opportunities.
• Knowledge of home care operations, payer sources, and referrals.

Responsibilities

• Build and nurture referral relationships with hospitals, facilities, and physicians.
• Conduct regular in-person facility visits, networking, and follow-up in assigned territories.
• Meet prospective clients and families, address needs, and move them toward care.
• Develop new referral sources and strengthen existing relationships for repeat referrals.
• Maintain an active referral pipeline and document outreach and conversions.
• Represent LifeKey Health professionally with healthcare professionals and families.
• Achieve 3–5+ qualified conversions per month after ramp period.
