Job summary

Humana's Inclusa team is seeking a Case Manager to serve Wisconsin Family Care members in Green/Lafayette counties. You will assess health and safety needs, develop a comprehensive Member Care Plan, and work with a Field Care Nurse to coordinate services in the least restrictive environment.

This fully remote role requires travel up to 40% and a valid driver's license. Typical hours are 8:00 am–4:30 pm CST, with a base pay range of $53,700–$72,600 per year and a comprehensive benefits package.

Qualifications

• Bachelor’s degree in human services or a related field required with 1 year of relevant experience.
• Alternative: Bachelor’s degree in another field with at least 3 years of relevant experience.
• Travel up to 40% with valid state driver’s license and proof of personal vehicle liability insurance with at least 100/300/100 limits.

Responsibilities

• Assess members' health and safety needs to create a comprehensive MCP.
• Coordinate with a Field Care Nurse to address needs per MCP.
• Travel to members' locations for assessments and visits.
• Arrange social, housing, and community support services.
• Maintain accurate documentation including MCP updates.
• Ensure cost-effective service delivery and risk education.
