Job summary

BronxWorks is seeking a full-time Case Manager in Bronx, NY to independently handle a caseload in a shelter-focused program. You will engage chronically homeless individuals through regular field visits, help develop goals and service plans, and connect clients with housing, health, and benefits resources.

The role requires a Bachelor’s degree in social work or related field, a valid driver’s license, and strong communication skills. Prior case management experience is preferred.

• Strong communication skills, both written and oral.
• Ability to engage with high-need populations and manage caseloads.

Responsibilities

• Independently manage a caseload.
• Conduct regular field visits to engage clients.
• Develop goals and service plans with clients.
• Provide referrals for mental health, medical care, housing, and benefits.
• Document interactions and monitor client progress.
• Coordinate care with providers and attend case conferences.
• Attend supervision sessions and drive agency vehicle when needed.
