Job summary

The Bridge in the Bronx is seeking a Senior Case Manager (Scattered Site) to provide supportive housing services to formerly homeless adults living in scattered-site apartments throughout the Bronx. You will conduct field visits, complete documentation promptly, and collaborate with clinicians and external providers to coordinate care.

Qualifications

• Bachelor's Degree Required with relevant experience.
• Experience working with mental health, crisis intervention, and managing trauma-triggered behavior.
• Excellent time management skills; stress-resistant, flexible; team player; open to feedback; creative; exceptional organizational skills; willingness to perform CPR/First Aid/AED/CPI/Naloxone.
• Preferred knowledge of AWARDS.

Responsibilities

• Deliver comprehensive case management services including housing assistance and service referrals.
• Coordinate with external providers and perform client assessments.
• Conduct fieldwork in clients' apartments and in community settings.
• Ensure timely completion of case notes within 48 hours.
• Provide crisis intervention when needed using de-escalation techniques.
• Report incidents and maintain census and documentation.
