Job summary

BronxWorks is seeking a dedicated Case Manager to join Park Haven, a supportive housing program in the South Bronx. You will manage a caseload, conduct home visits, and document progress toward treatment goals, working within a Housing First model to connect clients with essential services.

The role requires strong writing and communication skills, a Bachelor's in social work, and knowledge of city resources. Schedule is Mon-Fri 9am-5pm with on-site responsibilities.

Qualifications

• Bachelor's degree in social work or related field is required or preferred.
• One year of case management or relevant experience is preferred.
• Proficiency with Microsoft Office and basic business tech is required.
• Strong writing and oral communication skills are essential.
• Bilingual Spanish is highly desirable.

Responsibilities

• Maintain a caseload of 12-18 households with chronic homelessness history.
• Monitor and document client progress toward treatment plan goals.
• Conduct case conferences with clients and service providers as needed.
• Perform regular home visits and fieldwork when required.
• Assist with managing walk-in clients and navigation of resources.
