Responsibilities

Develop, assesses, and coordinates care management activities based on member needs to provide quality outcomes.
Develop or contribute to personalized care/service plans and educate members and families on options.
Identify providers, resources, and community services necessary for care.
Coordinate between members/families and care provider teams to ensure timely access to services.
Monitor progress and revise care plans as needed in collaboration with providers.
Collect, document, and maintain all member information for regulatory compliance.
