Job summary

Centene Corporation is seeking a field-based Care Manager in Oklahoma to assess member needs and coordinate care planning. The role requires collaboration with families, care teams, and community resources to improve health outcomes and access to services.

Qualifications

• Requires a Bachelor’s degree and 2–4 years of related experience.
• Nursing licensure in the applicable state is preferred for clinical roles.

Responsibilities

• Evaluate member needs, barriers to care, and resources to develop care plans.
• Coordinate between member/family and care team to ensure timely access to services.
• Provide psychosocial and resource support based on assessments and plans.
• Document and maintain member information to comply with regulations.
• Perform on-site visits to assess needs and collaborate with providers.
• Educate care managers and members on procedures, instructions, and benefits.
