Qualifications

• Equivalent experience acquired through applicable knowledge, duties, scope and skill.

Responsibilities

• Evaluates member needs, barriers to care, resources, and coordinates the plan for best outcomes.
• Develops/updates personalized care/service plans and identifies providers and community resources.
• Provides psychosocial and resource support to members and caregivers for accessing local services.
• Coordinates between member/family and care team to ensure timely access to services.
• Monitors progress toward goals and collaborates with providers for plan revisions.
• Documents and maintains member information to ensure regulatory compliance.
• May perform on-site visits to assess needs and collaborate with providers.
• Educates care managers and families on procedures, instructions, options, and benefits.
• Performs other duties as assigned and ensures adherence to policies.
