Job summary

Dot Cares, LLC seeks an experienced Care Manager for Westchester and Putnam Counties. This part-time, 1099 role is client-focused, leading assessments, care plans, and advocacy.

You will travel to client sites and coordinate with families and providers, with some remote coordination and admin work. Ideal candidates are RNs or Social Workers with strong organizational and communication skills, capable of independent case management, and willing to travel across Westchester and Putnam counties.

Qualifications

• Active NY State license as RN or LSW (LSW, LMSW, or LCSW).
• Experience in care management, case management, home health, hospice, hospital discharge planning, geriatrics, or social work.
• Strong communication, organization, and relationship-building skills.

Responsibilities

• Conduct assessments and develop care plans for assigned clients.
• Attend medical appointments and advocate for client needs.
• Coordinate communication among physicians, therapists, agencies, and facilities.
• Communicate with families and provide timely updates.
• Monitor client functioning and coordinate home care, transportation, and community resources.
• Maintain accurate case notes and organized files with confidentiality.
