Job summary

FreedomCare in Massachusetts seeks a Care Manager, RN to conduct in-home assessments, coordinate services, and educate patients and caregivers. The role involves frequent travel across the MA territory to ensure timely initiation and delivery of home-based care within AFC programs.

The ideal candidate has an active MA RN license, 2+ years of direct elder/adult care, and strong communication, documentation, and care-coordination skills to support positive patient outcomes.

Qualifications

• Current, unrestricted license as a registered nurse issued by the Massachusetts Board of Registration in Nursing.
• At least two years of recent experience providing direct care to elders or adults with disabilities, preferred.
• Tuberculosis screening completed before assignment and ongoing screening per CDC/MassHealth requirements.
• CORI, SORI, OIG exclusion, references, employment history checks completed.

Responsibilities

• Conduct initial in-home assessments and coordinate timely service activation.
• Develop, implement, and manage individualized plans of care; revise as needs change.
• Educate patients, caregivers, and internal teams on care needs and safety considerations.
• Coordinate patient care with physicians, internal partners, and community resources.
• Provide ongoing education to support caregiver competency and resource connections.
• Monitor and document changes in patient condition; escalate as needed.
• Assist with authorization, renewal, start-of-care, and continued-service processes.
• Maintain compliance with regulatory requirements and company policies.
