Job summary

Intent Clinical in Fairfield, CT, is seeking a part‑time Aging Care Manager to assess, plan, coordinate, and monitor care for elderly clients. You will collaborate with families and healthcare providers to ensure high-quality support and services across community, home, and remote settings.

The role requires a Master’s degree in a related field, licensure in good standing, and at least 5 years of care-management experience. A flexible schedule and independent work style are valued.

Qualifications

• Master’s degree in Social Work, Nursing, Gerontology, or related field.
• Professional licensure in good standing (RN, NP, LMSW or other nursing/social worker license).
• Minimum of 5 years of direct care experience in care management, home care, acute care, and/or a rehabilitation setting.
• Strong knowledge of aging issues, healthcare systems, and community resources.
• Experience working closely with families & professional providers.
• Ability to work independently and as part of a multidisciplinary team.
• Excellent written and oral communication, interpersonal, and organizational skills.
• Comfort and quick learning curve with technology on day‑to‑day tasks (i.e. Microsoft Office, Salesforce).
• Valid driver's license and reliable transportation.

Responsibilities

• Client Assessment and Care Planning: Assess needs, develop care plans, monitor and revise as needed.
• Care Coordination: Communicate between clients, families, healthcare providers and services.
• Resource Management: Connect clients with community resources and benefits.
• Monitoring and Follow‑Up: Conduct visits, adjust plans, document interactions.
• Family Support: Counseling, guidance on long‑term care planning and legal/financial issues.
• Facilitate family meetings and decision‑making processes.
